Transitional Care

Bridging the Gap from Hospital to Home

With an average length of stay less than 30 days, our transitional care services provide a care bridge between the hospital and home. Our primary goals are to offer excellent, compassionate care in a warm, hospitable environment dedicated to speeding your recovery home while working closely with you to prevent a relapse and to reduce the chance of rehospitalization. We design an accelerated rehabilitation care plan that is designed specifically to get you back to the activities and daily routines you enjoy as quickly as possible.

Our transitional care program offers:

  • Post discharge follow-up
  • In room or community dining
  • In room TVs with cable service
  • Comprehensive patient and family education
  • Beautifully appointed family areas and patient rooms
  • 24 hour coverage by nurses specially trained in patient assessment and evaluation
  • Large, spacious rehab gyms well equipped with state of the art medical and therapy equipment
  • Rehabilitation Services, including physical, occupational and speech language pathology therapies 6 days a week
  • Supervision by experienced physicians, other health care professionals and medical staff well versed in post acute care

Easy Referral, Admission and Discharge

Our streamlined processes for referral, admission and discharge for transitional care program enables each patient to quickly receive the care they need after they leave the hospital and then return home much faster when they recover.

Rehabilitation Care

Physical Therapy

Probably the most commonly known form of rehabilitation therapy, physical therapy is the treatment of disease or disability through therapy and exercise. Our programs include assistance with gait training, bed mobility, wound care, restraint reduction and wheelchair positioning.

Occupational Therapy

Occupational therapy is designed to help patients regain skills related to activities of daily living. Individuals may require this therapy if, as the result of an illness or injury, they have difficulty completing tasks such as eating, dressing, housekeeping or grooming.

Speech-Language Pathology

Speech therapists work with patients to help them regain or strengthen their ability to speak with ease and clarity. Our speech therapists utilize the latest technology and clinical processes to help patients regain the ability to communicate and improve swallowing.

Chronic Care

24 Hour Nursing Support

Our extended, long term care services are designed to offer 24 hour medical and support assistance for people with a chronic or terminal condition, disability, a prolonged illness, a serious injury or the challenges of aging. The need for extended care may only last for a few weeks or months, or may be needed for much longer. Typically, these patients or residents have medical conditions that require the expertise of an interdisciplinary team. The team includes the patient or resident’s attending physician, medical specialists, therapists, registered and licensed nurses, social workers, registered dietitians, life enrichment directors and nursing assistants.

Assistance is provided for:

  • Walking
  • Bathing
  • Dressing
  • Personal hygiene and grooming
  • Using the bathroom
  • Managing pain
  • Preventing unsafe behaviors
  • Preventing wandering
  • Eating
  • Medication management

Interdisciplinary Care

Care Centered Around You

A patient or resident in a Cascade facility is under the care of a physician, who visits regularly and is responsible for an overall plan of care. Once an individual is admitted to one of our facilities, the physician will write orders for all necessary medications and lead an interdisciplinary team of healthcare professionals in developing this individualized plan of care, including restorative and rehabilitation programs, any special dietary needs and clinical treatments.

Your disciplinary team will include:

  • Medical Director or attending physician
  • Consulting physician specialists
  • Nurse practitioner (in some locations)
  • Registered nurses
  • Licensed vocational nurses
  • Certified nursing aides
  • Physical, occupational and speech therapists
  • Social Worker
  • Certified dietician
  • Activity professionals
